Position Summary:
The Visiting-RN works collaboratively with the visiting NP; physicians; on-site with other interdisciplinary staff in the FNs and other members of the team, within 3 First Nations and the WFN-Safe Home. This will be accomplished by ensuring privacy and cultural safety for clients within the physical building of the First Nations’/Safe Home. This nurse will also consult with the physicians; nurse practitioners while following as applicable any/all medical directives of care allowed by the WFN-Health Department. The Visiting-RN will provide wholistic care to an individual clients or groups of clients including family members and significant others.

Job Duties & Responsibilities:
- Facilitate clients’ and visiting clinicians’ understanding of diagnoses, impacts, treatment and follow-up within primary care by consultation with NP, Physicians, and other nursing staff
- Complete client medical record within the WFN-EMR in an accurate and timely manner;
- Provide culturally relevant information to Indigenous elderly clients, families and health care team about clinics; their focus with outlined process for scheduling by assisting with linkages to traditional service provider or church/spiritual leader of their choice
- Working with all members of the team to ensure access to needed supports to ensure wholistic care is provided.
- All new admissions to each community will be questioned about admission vs engagement with this primary care providers; providing them with timely referral to a preferred provider; noting in their record, that the preference is to receive on-going services by this provider at this stie. Special conversation should be had about the location of their EMR record and that it is the responsibility of the funded provider (WFN) to ensure privacy and confidentiality.
- Perform other duties as necessary.
- Adhere to all policies and procedures of Wasauksing First Nation and any other relevant policies and procedures from external agencies and partners
- Perform other duties within the scope of the position as assigned by the Manager or Supervisor
Education & Experience:
- Possess a current College of Nurses of Ontario Certificate of Competence, having completed at the least a Registered Nursing Program at the college level accompanied by specialty certifications and/or an undergraduate degree in nursing or another area or an undergraduate degree in a recognized Ontario Nursing Program
- Membership with the Registered Nurses Association of Ontario
- Must have a minimum 5-years of previous related experience in a community care clinic or possess certification in primary care
- Must be knowledgeable with Telus-EMR, PS Suites
- Current CPR-Level C; AED; & First Aid
- Ability to speak, read and understand Anishnaabemowin (Ojibway Language) is an asset
Other Requirements:Â
- Ability to speak Ojibway would be an asset
- Valid Driver’s Licence and reliable personal vehicle
- Willing to work flexible hours as required, including evenings and weekends
- This position requires the individual to work with various families, youth and children. To protect children, youth and vulnerable person, WFN requires the individual to provide a vulnerable sector check/or criminal record check
