In this field-based role, you’ll conduct scheduled in-home Well-Child Visits, help identify health needs early, close care gaps, and keep kids connected to appropriate follow-up care.

Role Snapshot

• Pay: $120 per completed visit
• No-show pay: $45 for qualifying no-show visits
• Location: Maricopa County, AZ
• Work setting: In-home visits with local travel
• Schedule: Flexible; set your own schedule
• Focus: Pediatric Well-Child Visits and preventive care

What You’ll Do

• Conduct scheduled in-home Well-Child Visits throughout Maricopa County
• Review pediatric medical history, risk factors, vitals, and appropriate screening information
• Complete preventive and age-appropriate pediatric assessments
• Document visit findings accurately in the EMR
• Educate patients and families on recommended next steps
• Identify care gaps and coordinate appropriate follow-up
• Manage your local route without reporting to a clinic each day

Must-Haves

• Active Nurse Practitioner license in Arizona
• Registered/enrolled with Arizona Medicaid
• PNP-PC certification, or another NP certification appropriate to provide pediatric services within your population focus, education, national certification, and demonstrated competency
• Valid driver’s license and reliable transportation
• Comfortable working independently in patients’ homes
