We are seeking afull-time Transitional Healthcare Facilitator ( also known as a Re-Entry Coordinator) for needs at Rockville Correctional Facility in Rockville, Indiana.

Qualifications

•    Bachelor’s degree in psychology, social work, counseling or related field from an accredited institution required
•    Experience with/knowledge of community and state programs and services 
•    Two (2) years of experience in re-entry services or release planning preferred 
•    Must be able to pass background investigation and obtain agency security clearance where applicable
