Job Type: Full-time, field-based
Anticipated Start Date: January 2027, with onboarding beginning in December 2026
Compensation: Base salarydepending on licensure, certifications, clinical background, sales or liaison experience, and other qualifications plus bonus eligibility based on performance

INTEGRITY Home Care + Hospice, part of theBAYADA joint-venture footprint, is seeking a compassionate and proactive Transitional Care Manager to support patients and families throughout the Springfield market.
This is an opportunity to join a growing team in a brand-new role. You will help strengthen connections between hospitals, skilled nursing facilities, nursing facilities, patients, families, and the community-based services they need after a transition in care.
The role is open to both clinical and nonclinical professionals. Whether your background is in nursing, social work, nutrition, healthcare marketing, sales, case management, or facility relations, your ability to build trust and take initiative can make a meaningful difference.
What you’ll do
- Round at hospitals in the Springfield market, with required hospital presence at least three days per week.
- Visit hospitals, skilled nursing facilities, nursing facilities, and other healthcare partner locations throughout the market.
- Support field-based bedside and case-management activities.
- Build and maintain strong relationships with hospital teams, discharge planners, case managers, facility leaders, patients, families, and community partners.
- Help coordinate referrals and support smooth transitions from hospital or facility care to appropriate home- and community-based services.
- Educate patients, families, referral sources, and facility staff about available services and care options.
- Identify opportunities to strengthen referral relationships and improve transition processes.
- Communicate consistently with referral partners and internal teams regarding follow-up, needs, and next steps.
- Maintain accurate documentation and timely follow-through.
- Work independently in the field while collaborating closely with the local INTEGRITY team, Marketing Managers, and other Transitional Care Managers.

What we’re looking for
We welcome candidates from both clinical and nonclinical healthcare backgrounds, including experience as a:
- Registered nurse or other licensed healthcare professional
- Social worker or MSW
- Registered dietitian
- Hospital or skilled nursing facility liaison
- Healthcare marketer, account manager, or sales professional
- Case manager or discharge-planning professional
- Healthcare relationship or community outreach professional
The strongest candidates will:
- Be comfortable working in hospitals and healthcare facilities.
- Build relationships naturally and communicate with confidence.
- Demonstrate initiative, organization, and dependable follow-through.
- Be comfortable spending substantial time in the field.
- Enjoy a high-activity environment with frequent in-person interaction.
- Be willing to actively engage with referral partners and facility teams—not simply wait for opportunities to come to them.
- Adapt well while working independently and as part of a collaborative team.
