Employee Separation Policy:  Any past employee of UPH whether FTE, or PRN for the UPH float pool, cannot work travel in another Market no matter which facility they currently work at, or have worked at within the past 12 months.  If they worked travel at UPH (not FTE or PRN), the separation policy does not apply

HOLIDAY update:  unless the Unit is closed, your contractor is required to work 1-2 of 3 upcoming Summer holidays (Memorial Day, Independence Day, Labor Day)

• NBO:16 hrs
• EMR: EPIC
• OT/Holiday:1.15x multiplier
• Call Back:  1.5x multiplier with 2 hr CB minimum
• On Call: ***/hr

Billable OT: 

• 36-40 hrs/week, OT billable after 40
• 48 hrs/week, OT billable after 48

• Patient Types: Vascular procedure
• Call Team:  1 that covers both facilities (ILH/IMMC) with most emergencies at IMMC (level I)
• Duties:  scrub, circulate & monitor

• 12 months ofvascular procedure experience in a hospital or medical center setting [required]

• Work history/resume/employment history must have 12 months of current interventional vascular/IR lab/Cardiac Cath experience[required]
