Case Manager – Haven TorontoÂ
Position Type: Part-Time, PermanentÂ
Start Date: June 22, 2026Â
Schedule: 8am-4pm, Saturday and SundayÂ
Reporting to: Manager of Life Enrichment Programs Â
Compensation: $21,763.64/annumÂ
This position is part of the CUPE [Local # 2289-05] bargaining unit and is subject to the termsÂ
and conditions outlined in the collective agreement.Â
Since 1933, Haven Toronto has been a drop-in centre where elderly homeless, marginallyÂ
housed and socially isolated men can feel safe, where they can form and cultivate friendships,Â
and become part of a community. Our vision is a safe and just community in which elderÂ
homeless men can live a quality and dignified life.Â
Summary: The Case Manager provides client-centered support to individuals experiencingÂ
homelessness, substance use, mental health challenges, and complex social needs. Working asÂ
part of a multidisciplinary team, the Case Manager is responsible for coordinating care,Â
supporting access to services, and helping clients develop and achieve their goals throughÂ
individualized service planning. Â
Key ResponsibilitiesÂ
• Manage a caseload of clients with complex needs, providing ongoing case managementÂ
and supportÂ
• Complete assessments and service planning in collaboration with clientsÂ
• Provide supportive counselling, brief interventions, and crisis response as neededÂ
• Facilitate groups (e.g., life skills, harm reduction, wellness)Â
• Support access to housing, income supports, healthcare, and community resourcesÂ
• Maintain accurate and timely documentation on EMHware including case notes andÂ
service dataÂ
• Build and maintain relationships with community partners and service providersÂ
• Advocate for clients within systems and support navigation of servicesÂ
• Contribute to a safe, welcoming, and inclusive program environmentÂ
• Assist clients in accessing core services such as showers, laundry, meals, clothing, andÂ
recreation areas.Â
• Provide crisis intervention in accordance with Haven Toronto and SMH policies andÂ
procedures; document actions taken and recommendations for follow-up.Â
• Assist at the front desk by greeting clients, answering calls, registering clients for theÂ
program, booking appointments, providing referrals, and offering client support.Â
• Administer first aid or call emergency services when required.Â
• Work collaboratively with staff who work during the week in order to provide wrap
around care. Â
Knowledge, Skills, and Abilities
• Strong understanding of substance use, mental health, trauma, and recoveryÂ
• Knowledge of harm reduction and housing-first approachesÂ
• Understanding of the social determinants of health and barriers faced by individualsÂ
experiencing homelessness Â
• Demonstrated skills in assessment, case management, and supportive counselling Â
• Ability to work collaboratively within interdisciplinary teams and with communityÂ
partners Â
• Experience working within a trauma-informed and anti-oppression frameworkÂ
• Strong interpersonal, communication, and documentation skillsÂ
• Ability to manage competing priorities in a fast-paced environmentÂ
QualificationsÂ
• Bachelor’s Degree in Social Work (BSW) from an accredited institution. Â
• Registration and membership in good standing with the Ontario College of SocialÂ
Workers and Social Service Workers (OCSWSSW) is required.Â
• Demonstrated experience in case management, assessment, and working withÂ
individuals experiencing mental health and/or substance use challenges.Â
• Training in motivational interviewing and group facilitation is an assetÂ
• Additional language skills are an assetÂ
