Social Services and Admissions Liaison

Qualifications: 2 years of experience in long term care, licensed social worker or bachelor’s degree preferred, must possess excellent communication and organizational skills

Essential Job Functions:

• Oversee onboarding and training of all new admissions directors, discharge planners, and social workers.
• Develop one-to-one relationships with residents and families.
• Conduct monthly meetings and trainings for admissions and social services
• Quality Assurance responsibilities include conducting mock surveys and regular auditing.
• Manage all referral source accounts.
