Jefferson Health is seeking a compassionate and collaborative Palliative Care Physician to join our interdisciplinary team. The physician will provide comprehensive palliative care services to patients with serious, complex, and life-limiting illnesses, working closely with patients, families, and multidisciplinary care teams to improve quality of life throughout the course of illness.The Palliative Care Physician will provide expert symptom management, facilitate goals-of-care discussions, support complex medical decision-making, and assist patients and families in navigating serious illness. The physician will collaborate with physicians, advanced practice providers, nursing, social work, chaplaincy, care management, and other healthcare professionals to develop individualized plans of care aligned with each patient’s needs, goals, and values.
Job Description
Responsibilities include providing inpatient and/or outpatient palliative care consultation, managing pain and other complex symptoms, conducting goals-of-care and advance care planning discussions, supporting patients and families during transitions of care, and collaborating with primary and specialty care teams. Opportunities may also be available to participate in education, quality improvement, program development, and teaching of medical students, residents, fellows, and other healthcare professionals.
Qualifications:
MD or DO degree from an accredited medical school
Completion of an accredited residency program
Fellowship training in Hospice and Palliative Medicine preferred
Board certified or board eligible in Hospice and Palliative Medicine
Eligible for medical licensure in the applicable state
Strong communication and interpersonal skills, particularly in complex and sensitive clinical situations
Commitment to interdisciplinary, patient- and family-centered care
The successful candidate will demonstrate a commitment to providing compassionate, evidence-based care while working collaboratively across disciplines to support patients and families facing serious illness.
