  • Guide antenatal and postnatal care, newborn and child health, immunization linkages, nutrition screening and counselling, and referral activities in line with applicable guidance.

  • Provide technical support to Regional Health Specialists and facility, hub, and spoke teams to address identified service-delivery and quality gaps.
  • Promote low-dose, high-frequency (LDHF) learning where appropriate, alongside regular coaching, supportive supervision, and continuous quality improvement.
  • Use performance data and supervision findings to identify service gaps, agree corrective actions, and monitor follow-up.

3. Implementation and Budget Performance

  • Monitor delivery of assigned health activities against approved milestones, outputs, targets, timelines, and budgets.
  • Coordinate with relevant health specialists and Finance, Sourcing, and MEAL colleagues on training, supervision, supplies, community activities, and other approved implementation inputs.
  • Review implementation progress against resource utilization and flag delays, variances, risks, or forecast changes to the Grant Program Manager / FCI Project Manager.
  • Ensure technical activity plans and documentation support program reporting, resource tracking, and financial accountability.

4. Community, Facility, Referral, and Continuity-of-Care Linkages

  • Strengthen bidirectional referral and feedback pathways among communities, facilities, spokes, and hubs.
  • Support community case finding, HIV/TB contact tracing, adherence and treatment follow-up, and community follow-up for maternal, newborn, child health, and nutrition services.
  • Identify barriers to access, linkage, retention, and continuity of care, and coordinate corrective action with field and hub-and-spoke teams.
  • Promote identification and referral of priority and vulnerable clients and support effective linkages between community and facility services.

5. Stakeholder Coordination, Reporting, and Learning

  • Coordinate with the Ministry of Health (MoH), regional health authorities, hubs, faith-based organization (FBO) partners, and consortium technical teams.
  • Contribute to national and regional technical reviews and donor and consortium reporting.
  • Document lessons, implementation adaptations, and good practices relevant to quality, sustainability, scale-up, and transition.
  • Promote collaboration across technical areas to support integrated planning, implementation, and problem-solving.

Qualifications and Experience
