Role Overview

As a Team Ldr-CCS RN, you will coordinate clinical workflow and oversee the daily activities of a Community Care Services nursing team, such as Hospice or Home Health Care. You¿ll ensure safe, effective, and compliant patient care while serving as a clinical resource and liaison between leadership and staff. Your role is vital to maintaining high standards of care and supporting both your team and the patients we serve.

What You Will Do

  • Coordinate clinical workflow and oversee daily activities of an assigned CCS nursing team.
  • Ensure safe, effective, and compliant delivery of patient care within the assigned program.
  • Serve as a clinical resource and liaison between leadership and staff.
  • Utilize nursing principles to assess and address patient needs.

What You Will Need

Required Qualifications

  • Graduation from an accredited Registered Nurse program.
  • Minimum one (1) year of progressively responsible nursing experience in applicable specialty area.
  • Current licensure by the State of Michigan as a Registered Nurse.
  • Strong clinical assessment and leadership skills.
  • Ability to coordinate workflow and manage competing priorities.
  • Effective interpersonal and communication skills.
  • Commitment to customer service excellence.
  • Self-directed, flexible, collaborative, and respectful of diversity.

Preferred Qualifications

  • Specialty certification preferred where applicable.
