Pleasant Hill Post Acute is seeking a compassionate, dependable, and clinically skilledRegistered Nurse (RN) to join our team on aPart-Time / On-Call basis.

The ideal candidate is committed to providing high-quality, resident-centered care and working collaboratively with residents, families, physicians, and the interdisciplinary healthcare team.

Responsibilities

• Provide skilled nursing care to residents in accordance with individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and promptly report changes in health status.
• Perform nursing assessments and assist with care planning.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with residents, families, physicians, and healthcare staff.
• Follow infection control, safety, and facility policies and procedures.
• Promote resident dignity, comfort, safety, and overall well-being.
• Respond appropriately to changes in residents' conditions and emergency situations.

Qualifications

• Current and validCalifornia Registered Nurse (RN) license.
• Previous skilled nursing or post-acute experience preferred.
• Strong clinical, communication, and organizational skills.
• Reliable, compassionate, and team-oriented.
• Ability to work independently and effectively in a fast-paced healthcare environment.
