RN/LPN Nurse Case Manager _ Work from Home

HealthWorks Medical is hiring an experienced RN or LPN Nurse Case Manager to join our Marine Medical Management team!

Enjoy a Monday–Friday, 8 AM–5 PM schedule with the opportunity to work fully remote after completing onsite training.

Job Description Report:

•  Acts as patient advocate in delivery and coordination of patient care.
•  Able to manage a large number of patient’s cases with high energy and organization
• Collaborates with other health care providers and Medical Director regarding patient needs.
•  Review and assess medical records to determine employee’s ability to perform job duties
•  Respond to workplace injury or illness by triaging initial telehealth call and providing follow up care.

Qualifications

Required Skills:

•  Ability to work independently and make decisions
•  Ability to effectively communicate with all levels of management, staff and partnering companies
•  Ability to organize, perform and prioritize multiple functions and tasks
•  Ability to organize, attend and participate in staff development, training, and wellness programs
•  Strong technical skills and the ability to adapt to new healthcare technology systems.

Minimum Education- Associates Degree or; one or two years related experience
and/or training; or equivalent combination of education and experience.
Required Licenses [Kentucky, United States] RN or LPN

•  Registered Nurse with a current KY license or valid compact state license OR
•  LPN with a current KY license or valid compact state license
•  BLS certification - American Heart Association (Healthcare Provider) or American

Red Cross (Healthcare Provider) required, must be current.
