Resource Specialist

Pay Range:$25/hr - $27/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Assess member needs and contribute to care/service plans in collaboration with the care team.
• Connect members to local resources, assisting with documentation and application processes.
• Serve as a resource for members seeking support, determining eligibility based on assessments.
• Monitor service delivery and follow up to ensure timely access to care.
• Collaborate with providers and community organizations to facilitate access to essential services.
• Develop educational materials and training programs for care management staff.
• Maintain accurate documentation of resource services and ensure information is up to date.
• Conduct outreach to locate hard-to-reach individuals and families in the community.
• Perform other duties as assigned to meet member and business needs.

Key Requirements and Technology Experience:

• A Bachelor's degree and2–4 years of related experience in child welfare and pediatric case management.
• Hands-on experience connecting children and families to local community resources.
• Knowledge of foster care, adoption, and health plan services is preferred.
