Resource Specialist

Pay Range:$25/hr - $27/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Assess member needs and contribute to care/service plans in collaboration with the care team.
• Connect members to local resources for services including employment, housing, and foster care.
• Act as a resource for members seeking local services, determining eligibility based on assessments.
• Monitor the delivery of care/services and follow up to ensure timely access to appropriate resources.
• Collaborate with providers, community organizations, and governmental entities to support families.
• Assist in developing educational programs for staff to improve resource identification.
• Ensure all resource service information is documented and up to date.
• Conduct outreach to locate hard-to-reach individuals and families in the community.
• Perform other duties as assigned to meet member and business needs.

Key Requirements and Technology Experience: 

• A Bachelor's degree and2–4 years of related experience in child welfare and pediatric case management.
• Hands-on experience connecting children and families to local community resources.
• Knowledge of foster care, adoption, and health plan services is preferred.
