Pay Range: $20 - $22/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Provide outreach and support to women participating in the Planning for Healthy Babies program.
• Educate and mentor members throughout their 12-month postpartum period.
• Ensure timely completion of SOPs aligned with outreach metrics.
• Contribute to improving postpartum care and support for new mothers.

Key Requirements and Technology Experience:

• Experience in maternal and child health programs.
• Familiarity with HEDIS measures.
• 2+ years of experience in a related field.
• High School Diploma or GED.
• Valid Driver License.
• Strong communication skills.
• Excellent time management abilities.
