Position Summary
Pagosa Springs Medical Center is seeking aRepresentative: Health Information Management (HIM) to support the daily operations of the HIM department.
This position serves as a primary reception and service point for patients, visitors, providers, and staff. Key responsibilities include assisting withRelease of Information (ROI) requests, document scanning and indexing, medical record maintenance, and ensuring the accuracy, confidentiality, and integrity of patient records.
The successful candidate will demonstrate excellent attention to detail, strong organizational and customer service skills, the ability to prioritize competing responsibilities and meet deadlines, and a high degree of professionalism when working with sensitive patient information.

Key Responsibilities
Reception & Customer Service
- Greet and assist patients, visitors, physicians, and staff in a professional and courteous manner
- Answer incoming telephone calls and respond to questions regarding medical records and HIM services
- Provide excellent customer service while handling sensitive and confidential information
- Maintain patient confidentiality in accordance with HIPAA and PSMC requirements
Release of Information (ROI)
- Receive, review, and process applicable requests for medical records
- Assist patients who present to the department with record requests
- Process STAT requests as assigned
- Verify patient identity and authorization before releasing protected health information
- Ensure records are released in accordance with HIPAA, applicable regulations, and organizational policies
- Maintain accurate documentation of ROI activities
- Track, log, and assist with completing requests within established turnaround times
Document Scanning & Medical Record Management
- Prepare paper documents for scanning by organizing and sorting records
- Scan, index, and import medical records into the electronic health record
- Verify image quality and ensure documents are indexed to the correct patient record
- Identify and correct scanning errors
- Reconcile missing or incomplete documentation
- Follow established document retention and destruction requirements
General HIM Support
- File, retrieve, and maintain medical records as needed
- Support provider and departmental requests for patient records
- Maintain department logs, reports, and tracking systems
- Verify the accuracy of medical records before release
- Maintain patient confidentiality at all times
- Prioritize assigned work and complete responsibilities within established deadlines
- Maintain competency through education, training, and in-service activities
- Support positive communication and working relationships throughout PSMC
Qualifications
Required Qualifications
Education
- High school diploma or equivalent
Skills & Certification
- Current AHA BLS Heart Saver certification
- Knowledge of Microsoft Office, Access, data entry, and spreadsheets
- Strong customer service and communication skills
- Strong attention to detail and organizational skills
- Ability to maintain confidentiality and appropriately handle sensitive information
- Proficiency with computers, scanners, and office software
- Ability to prioritize responsibilities and work independently in a fast-paced environment
Preferred Qualifications
- Previous experience in medical records, Health Information Management, a healthcare office, or an administrative setting
- Experience with electronic health record systems
- Experience with document imaging and scanning processes
- Knowledge of Release of Information procedures and HIPAA regulations
- Additional language skills

Schedule & Work Environment
This is afull-time, non-exempt position, anticipated to work40 hours per week.
Work is performed in an office setting and includes frequent daily communication with patients, families, employees, vendors, and members of the public. The environment may include frequent interruptions and requires the ability to remain organized, professional, and attentive to detail.
The position involves repetitive motion and periods of prolonged sitting, as well as routine use of computers, scanners, and other office equipment.
PSMC cross-trains this position to effectively perform the duties ofClerk: ROI. This position doesnot supervise other personnel.

- Placement within the range will be based on the selected candidate's qualifications, relevant experience, and PSMC's applicable compensation practices
