Job Description

Registered Practical Nurse- Part timeĀ 
Mission: To provide high quality resident centered care in a caring and responsible manner with the available resources.Ā 
Reporting to the Executive Director of Nursing (EDON) or Director of Nursing (DON), theĀ 
Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented,Ā 
goal-directed nursing care in accordance with the provincial standards of care.Ā 
Key Responsibilities:Ā 
• To work as a member of the inter professional team as described by F. J. Davey HomeĀ 
Professional Practice Model applying the person centered approach to care;Ā 
• To complete and ensure resident's clinical care assessments, care/service plans andĀ 
documentation are accurate, up to date and changes communicated as required;Ā 
• To implement and evaluate the effectiveness of the care/service plans and makeĀ 
changes as required;Ā 
• To assist with scheduling of resident's care conferences and participate in conferencesĀ 
where applicable;Ā 
• To work closely with EDON/DON to facilitate resident's move in, move out and transfers;Ā 
• To provide direct resident care and medication/ treatment administration and documentĀ 
care in compliance with regulatory and F. J. Davey Home Care standards and policiesĀ 
and procedures;Ā 
• To process the orders and safe keep drugs and narcotics and administer in complianceĀ 
with all regulatory requirements, scope of practice and F. J. Davey Home facility policies andĀ 
procedures;Ā 
• To facilitate and provide resident/family education;Ā 
• To plan, coordinate, manage & evaluate the resident's care plan working with theĀ 
resident/substitute decision maker, physicians, nursing & interdisciplinary team andĀ 
contracted service providers;Ā 
• To coordinate nursing team's resident care assignments to ensure efficient and timelyĀ 
delivery of services to residents;Ā 
• To respond to and manage significant changes in resident's condition and or medicalĀ 
emergencies; provide emergency first aid;Ā 
• To teach, delegate to, and direct unregulated care providers;Ā 
• To respond to resident/family concerns and ensure appropriate action taken and orĀ 
concern communicated to EDON/DON and other departments;Ā 
• To follow and assist the EDON/DON in the coordination, implementation and monitoring ofĀ 
Infection Prevention and Control , Health and Safety and Clinical Programs andĀ 
Quality/Safety Plans for the Nursing Department;Ā 
• To uphold and promote the organization's values and philosophy relating particularly toĀ 
ethics, morality, and integrity as set out in F. J. Davey Home Code of Conduct;Ā 

• To complete all other duties as assigned.Ā 
Qualifications:Ā 
• Current registration as a Registered Practical Nurse in Ontario;Ā 
• Experience working in a senior's environment and with unregulated care providersĀ 
preferred;Ā 
• Education and experience in gerontology, rehabilitation, community/complex/long termĀ 
care an asset;Ā 
• Must possess strong written and oral communication, critical thinking, care coordination,Ā 
time management and customer focus skills;Ā 
• Must have demonstrated sound clinical knowledge;Ā 
• Current Basic Cardiac Life Support and First Aid certification required;Ā 
• Computer proficiency and experience with Microsoft office applications.Ā 
• Current (within 6 months) Vulnerable Position/Sector Screening (VPS).Ā
