Job Summary

Eminence Home Care is seeking a compassionate and dedicatedRegistered Nurse (RN) to join our growing home care team. The RN will be responsible for conducting patient assessments, developing individualized care plans, supervising caregivers, and ensuring high-quality care is delivered in accordance with state regulations and company standards.

Key Responsibilities

• Conduct initial and ongoing patient assessments in the home.
• Develop, implement, and update individualized patient care plans.
• Perform supervisory visits to evaluate caregiver performance and patient satisfaction.
• Monitor patients' health conditions and identify changes in care needs.
• Provide education and guidance to patients, families, and caregivers.
• Complete accurate and timely clinical documentation.
• Ensure compliance with state regulations, agency policies, and healthcare standards.
• Collaborate with caregivers, families, physicians, and office staff to coordinate patient care.
• Assist with caregiver training, competency evaluations, and clinical support as needed.
• Address patient concerns and provide recommendations to improve quality of care.

Qualifications

• Active and unrestricted Registered Nurse (RN) license in the applicable state.
• Minimum of 1 year of nursing experience preferred.
• Previous home care or home health experience preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and manage multiple patient cases.
• Valid driver's license and reliable transportation.
• Proficiency in electronic medical records (EMR) and clinical documentation.
