Position Summary:
The Post-Acute Network Performance Specialistis responsible for facility-level performance engagement and operational partnership acrossLuminis Health’s post-acute network.
This role serves as the hospital-designated performance liaison to Skilled Nursing Facilities (SNFs)and, as phased, Inpatient Rehabilitation Facilities (IRFs), Long-Term Acute Care Hospitals (LTACHs), and other post-acute partners.
The Specialist supports execution of the enterprise post-acute strategy by conducting structured performance reviews,identifying operational barriersimpacting throughput and readmissions, andfacilitating resolution pathways in coordination with internal and external stakeholders.
This positionoperates at the partner-performance level and does not perform inpatient discharge planning, placement decisions, or care management workflows.
Essential JobDuties:
Conduct Facility-Level Performance ReviewsÂ
Lead recurring structured performance discussions with assigned post-acute partners to review throughput, readmissions, referral patterns, and operational reliability metrics.Monitor Partner Performance TrendsÂ
Analyze facility-specific data toidentify emerging patterns related to acceptance timeliness, cycle time variability, readmission trends, and escalation frequency.Support Barrier Identification and ResolutionÂ
Identify recurring operational or clinical barriersimpacting patient transitions and coordinate structured resolution plans in collaboration with partner leadership and internal stakeholders.Facilitate Escalation CommunicationÂ
Serve as the first-line external liaison for performance-related concerns, ensuringtimely documentation, communication, and follow-up of identified issues.Promote Throughput ReliabilityÂ
Work with post-acute partners to improve referral responsiveness, documentation alignment, authorization readiness, and discharge progression reliability.Support Preferred Network Performance StandardsÂ
Reinforce established performance expectations and tiering criteria through routine engagement and performance transparency.Strengthen Clinical Alignment ConversationsÂ
Collaborate with facility clinical leadership to review readmission diagnoses, high-impact DRG trends, and transition breakdowns that mayimpact system performance.Maintain Structured Performance DocumentationÂ
Track meeting outcomes, barrier logs, corrective actions, and performance improvement initiativesin accordance with departmental governance standards.Provide Intelligence to Manager and DirectorÂ
Escalate systemic trends, recurring patterns, and high-variance performance issues to leadership to inform enterprise-level decision-making.Support Network Expansion and Governance ScalingÂ
Assist with onboarding of new partner facilities into the governance model as the network expands across SNF, IRF, LTACH, and home-based settings.
Qualifications/Experience Requirements:
Bachelor’s degree in Healthcare Administration, Public Health, Social Work, or related fieldrequired.
Minimum of 3–5 years of experience in post-acute care, care management, hospital operations, or healthcare performance improvement.
Demonstrated experience in one or more:
Post-acute strategy
Population health
Care continuum operations
Value-based payment models
Healthcare network development

Experience in Maryland’s TCOC or similar global budget environmentstrongly preferred.
Proven ability to influence executive and external partner stakeholdersÂ
High ethical standards and a commitment to compliance
Excellent organizational, time management and problem-solving skills
Ability to function effectively as part of a multi-disciplinary team
