Pennant Services is seeking an experienced Registered Nurse to join our Population Health team as a Population Health Coding & Documentation Integrity Nurse Specialist/Auditor.
This is a unique opportunity for a clinically experienced nurse who enjoys digging into the medical record, understanding the full picture of a patient's health, collaborating with providers, and using data and clinical documentation to improve population health. This isn't a traditional bedside nursing role, and it isn't simply a medical coding position.
Instead, you'll bring the two worlds together. You'll use your nursing knowledge and clinical judgment to review medical records, identify documentation opportunities, support accurate ICD-10-CM and risk-adjustment practices, audit documentation and coding, and help physicians and advanced practice providers accurately capture the complexity of the patients they care for.
What You'll Do
- Review patient medical records to determine whether documentation accurately and completely reflects the patient's health status and chronic disease burden.
- Conduct prospective and retrospective clinical documentation and coding reviews.
- Identify documentation gaps, specificity opportunities, and conditions that may require provider clarification.
- Review ICD-10-CM diagnoses and supporting clinical documentation for accuracy and completeness.
- Support HCC and other applicable risk-adjustment documentation initiatives.
- Perform focused documentation and coding audits and summarize findings.
- Identify trends across providers and patient populations.
- Partner directly with physicians, nurse practitioners, physician assistants, nurses, coders, and population health team members.
- Educate providers on clinical documentation, coding specificity, and risk-adjustment concepts.
- Develop practical provider education, tip sheets, and documentation resources.
- Research coding, documentation, CMS, and regulatory guidance when questions arise.
- Help improve workflows that make accurate clinical documentation easier for providers.
- Support quality, compliance, and value-based care initiatives across Pennant.

Required
- Active, unrestricted Registered Nurse license
- Graduate of an accredited nursing program
- At least 3 years of professional nursing experience
- Strong clinical knowledge, including medical terminology, anatomy, physiology, pathophysiology, and chronic disease
- Experience reviewing and interpreting medical records
- Working knowledge of ICD-10-CM and clinical documentation principles
- Strong critical-thinking and analytical skills
- Exceptional attention to detail
- Strong written and verbal communication skills
- Comfort working directly with physicians and other clinicians
- Ability to work independently while collaborating effectively in a multidisciplinary environment
We'd Especially Like to Meet You If You Have
- Clinical Documentation Integrity/Improvement (CDI) experience
- Population health or value-based care experience
- Medicare Advantage or risk-adjustment experience
- Experience with CMS-HCC documentation and coding
- Coding or clinical documentation auditing experience
- Provider education experience
- Experience with prospective and retrospective chart review
- Experience caring for medically complex or chronically ill populations
- A BSN
- CPC, CRC, CDIP, CCDS, or another relevant documentation/coding credential

Work Location
- This is a full-time, 40-hour-per-week hybrid position based in Utah.
- You'll work onsite at our Draper, Utah office at least one day each week, with the remaining scheduled days generally worked remotely.
- Additional in-person work and periodic travel will be required based on business needs, including provider education, meetings, audits, training, and collaboration with Pennant teams and affiliated operations.

The hourly range for this position is $38.00-$53.00 per hour. Actual hourly rate is dependent upon experience.
