Position Summary:
As a member of an interdisciplinary primary care team within a First Nation you will be providing comprehensive, client-centred primary care which emphasizes as well access to Traditional Indigenous Healers, health promotion, illness prevention, transportation and continuity of care for our clients.
Overall responsibilities include the observation and assessment of episodic and chronic health care needs of the clients across the lifespan, promoting wholistic approaches and providing comprehensive comprehensive health examinations and treatment to assist individuals, families and groups achieve an optimal state of health and well-being

Job Duties & Responsibilities:
- Facilitate clients’ and visiting clinicians’ understanding of diagnoses, impacts, treatment and follow-up
- Complete all observations, assessments, review client medical history and record within the WFN-EMR in an accurate and timely manner;
- Provide culturally relevant information to Indigenous elderly clients, families and health care team about clinics; their focus with outlined process for scheduling by assisting with linkages to traditional service provider or church/spiritual leader of their choice
- Working with all members of the team to ensure access to needed supports to ensure wholistic care is provided.
- All new admissions to each community will be questioned about admission vs engagement with this primary care providers; providing them with timely referral to a preferred provider; noting in their record, that the preference is to receive on-going services by this provider at this stie. Special conversation should be had about the location of their EMR record and that it is the responsibility of the funded provider (WFN) to ensure privacy and confidentiality.

Education & Experience:
- Medical degree and current license to practice medicine in the province of Ontario
- Ability to function as a member of a multi-disciplinary team (include primary care, mental health, home and community care, traditional wellness/treatment with a high level of respect, professionalism, co-operation and cultural sensitivity
- Excellent interpersonal and communication skills with an innate ability to interact positively with Indigenous peoples, management and knowledge and understanding of issues impacting the Indigenous population.
Other Requirements:Â
- Valid Driver's License and reliable personal vehicle
- Willing to work flexible hours as required, including evenings and weekends
- This position requires the individual to work with various families, youth and children. To protect children, youth and vulnerable persons, Wasauksing First Nation requires the individual to provide a vulnerable sector. Or criminal record check
