About the Role

PharmD Live is seeking aPhysician Assistant (PA-C) or Nurse Practitioner (NP) to support patients through ourChronic Care Management (CCM) andRemote Patient Monitoring (RPM) programs.

This role focuses on proactive, patient-centered care, chronic disease management, clinical assessment, patient education, and coordination with our multidisciplinary care team.

Key Responsibilities

• Conduct clinical and health-risk assessments for patients with chronic and complex conditions.
• Develop, manage, and update individualized care plans.
• Monitor patient progress and RPM data and identify concerns requiring intervention.
• Provide patient education, coaching, and support related to chronic disease management and medications.
• Coordinate care with physicians, pharmacists, nurses, social workers, and other providers.
• Escalate clinical concerns and facilitate appropriate follow-up when needed.
• Maintain timely, accurate, and compliant EHR documentation.
• Support CCM/RPM quality, compliance, and patient engagement initiatives.

Qualifications

Required:

• Active, unrestrictedCalifornia PA-C or NP license.
• Current national certification (NCCPA, AANP, or ANCC, as applicable).
• 2+ years of clinical experience in primary care, internal medicine, geriatrics, chronic disease management, or a related setting.
• Strong clinical judgment, communication, and patient-care skills.
• EHR proficiency and ability to work independently in a remote environment.
• Reliable internet and a private, HIPAA-compliant workspace.

Preferred:

• Experience with CCM, RPM, telehealth, population health, or value-based care.
• Experience managing patients with multiple chronic conditions.
• Familiarity with digital health and remote monitoring platforms.
