Position Overview

PharmD Live is seeking a detail-oriented, patient-focused Medical Assistant to support our telehealth care management program. This role combinespatient outreach, appointment scheduling, care coordination, and core Medical Assistant support in a remote, high-volume environment.

Key Responsibilities

• Conduct outbound and inbound patient calls and schedule appointments
• Support Annual Wellness Visits and care management outreach
• Perform medication reconciliation and update patient information
• Document patient interactions accurately in the EHR
• Prepare charts and support care coordination and follow-up
• Maintain HIPAA compliance and escalate clinical concerns appropriately
• Meet established outreach, scheduling, and documentation targets

Qualifications

• Medical Assistant certification or equivalent healthcare experience required
• Prior healthcare, patient outreach, scheduling, or care coordination experience
• Strong communication, organization, and documentation skills
• Comfortable with high-volume calling and performance metrics
• EHR experience preferred
• Medicare patient experience preferred
• Reliable and self-motivated in a remote environment
