Personal Care Clinical Coordinator at Landis Homes Personal Care
Position Essential Duties/Responsibilities:

•Works closely with the Director of Personal Care, team leads and alternate shift RN
    Campus Supervisors for effective 24-hour management.
•Supervises, motivates, trains, and defines the organization's standards and communicates
    effectively with staff using team leadership concepts.
•Completes One 2 One Conversations with team members in a timely fashion, minimum of annually.
•Relates appropriately with other departments and consultants when dealing with multi-disciplinary
    issues.
Reviews and updates PEER (the electronic incident reporting system) Monday-Friday, ensuring all incidents are followed up timely and appropriately.
•Recognizes and responds to significant changes in resident condition and provides leadership in
     emergencies.
•Rounds daily on each house, ensuring level of care needs are met.
•Provides clinical support to the team members.
•Oversees quality issues, monitors incidents/accidents, provides follow-up monitoring, and
    addresses issues immediately.
Responsible for updating Clinical Personal Care Policies, minimally on an annual basis or with regulatory changes.
Completes clinical PC Quality Improvement (PCQI) audits and reports through the PCQI Committee quarterly.
•Performs with a high degree of exactness and has good time management skills.
•Works with LH Infection Control Preventionist in monitoring infection control issues in
    assigned house(s), completes forms and reviews reports.
•Understands and promotes the concept of Personal Care. Encourages team members to promote
    resident independence and maintain the highest level of functioning.
•Assists with direct care, such as bathing, assists in the dining room, responds to call bells and
    participates in resident activities as needed.
•In consultation with the Director of Personal Care and Social Services, meets with families as
    needed and responds to concerns and issues as soon as possible. Report concerns through the PEER electronic incident reporting system for timely follow-up.
•Serves as a change agent, communicating change positively to staff.
•Ensures completion, accuracy, and compliance of RASP’s and DME’s.
•Is available (on-call) in an emergency situation when required staffing is affected.
•Shares on-call with the Director of Personal Care and other PC leaders, this is about a every 4 week rotation.
•Works with scheduler on staffing issues to ensure that call-offs and vacancies are filled to provide
    adequate coverage as per regulatory requirements and resident requirements.
•Participates in quarterly PC Quality Improvement meetings, and reports on trends.
•Coordinates monthly billing assessments with Personal Care households, and submits billing to the Finance department.
•Maintains current CPR certification.
•Relates well with residents, clients, family members, staff and the community.
•Adheres to dress code required for role.
•Understands and models the Honoring Lives principles in leadership, relationships and customer
    service.
•Provides and supports growth and development opportunities for team members.
•Communicates calmly and gently with residents, family members and staff, exhibiting patience,
    compassion, understanding and sensitivity.
Complete Dementia Friendly training within first year. (Lumina Mentor, Lumina Advisor)
•Participates in training and development opportunities as offered. Assists with establishing training schedule/plan and training clinical team members.
•Demonstrates dependability, competence, accuracy and is observant.
•Adheres to compliance policies regarding confidentiality, HIPAA regulations, and resident and client
    rights.
•Complies with regulatory agency standards and applicable policies and procedures.
•Complies with safety and infection control protocols in all aspects of the job.
•Demonstrates understanding of the Code of Conduct and abides by its guidelines in all aspects of
    the job.
•Additional duties and responsibilities as assigned.
Qualifications
LPN Qualifications
•Current license in good standing as a Licensed Practice Nurse in the Commonwealth of Pennsylvania required.
•Prior supervisory experience preferred.
•Personal Care Home Administrator license preferred, must obtain within 2 years of hire
•Knowledgeable of Pennsylvania State Personal Care Home Regulations.
•Proficient in use of computer and related technology required for the role.
•Good verbal, non-verbal and written communication skills required.
•Demonstrates leadership, supervisory skills, dependability and a positive demeanor.
