KeyResponsibilities

• Evaluatepatientsanddevelopindividualizedtreatmentplans.

• Provideskilledphysicaltherapyservicesinaccordancewithphysicianordersandcareplans.

• SupervisePhysicalTherapistAssistants(PTAs)incompliancewithstatepracticeregulations.

• Collaboratewithinterdisciplinaryteammemberstocoordinateandoptimizepatientcare.

• Participateincareconferences,rehabilitationmeetings,andutilizationreviewsasneeded.

• Maintaintimelyandaccuratedocumentation,includingevaluations,treatmentnotes,progressreports,anddischargesummaries.

• Educatepatients,families,andcaregiversontreatmentgoals,mobilitystrategies,safetytechniques,andhomeexerciseprograms.

• Assistwithdischargeplanningandrecommendationsforcontinuedcare.

• Recommendandcoordinateappropriatedurablemedicalequipment(DME).

• Ensureasafetreatmentenvironmentbymonitoringandreportingequipmentmaintenanceneeds.

Qualifications

• CurrentPhysicalTherapist(PT)licenseingoodstanding.

• Strongcommunication,organization,andinterpersonalskills.

• Passionfordeliveringexceptionalpatient-centeredcare.

• Abilitytoworkeffectivelywithinamultidisciplinaryteam.

• Newgraduatesareencouragedtoapply.
