Your Role as a Home Health PTA (Physical Therapist Assistant)

As aPhysical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing onrehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Providein-home physical therapy services following the established Plan of Care
• Assist patients withtherapeutic exercises, mobility training, and rehabilitation programs
• Supportsenior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers onhome exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation usingWellSky Kinnser EMR

Qualifications:

• ActiveCalifornia Physical Therapist Assistant (PTA) license
• CurrentCPR certification (AHA required)
• Minimum1 year of clinical experience (home health experience preferred)
• Strong interest inhome health, rehab, and senior care
• Excellent communication and organizational skills
• Valid driver’s license, auto insurance, and reliable transportation
