Home Health RN Responsibilities

• Provideskilled nursing care in patient homes, including assessments, wound care, medication management, and treatments
• Develop and implement individualizedhome health care plans
• Monitor patient progress and adjust care plans as needed
• Educate patients and families ondisease management, rehab, and senior care
• Coordinate care with physicians, therapists, and theDirector of Nursing
• Document patient visits in compliance with home health regulations

Qualifications – Home Health RN Jobs

• ActiveCalifornia Registered Nurse (RN) license (required)
• Minimum1 year RN experience (home health experience preferred)
• Experience inrehab, geriatric care, or community health is a plus
• Strong clinical, communication, and organizational skills
• Valid driver’s license and reliable transportation
• Ability to travel throughout Orange County

Preferred Qualifications

• Home Health RN experience
• Public Health Nurse (PHN) certification
• Experience withmobile therapy or home-based care models
