Medical Social Worker (MSW) – Home Health

About the role

The Medical Social Worker assesses psychosocial needs and provides home health social services under the plan of care, coordinating resources that support safe and effective care.

What you will do

Complete psychosocial assessments, provide counseling and resource coordination within scope, document services timely, communicate with the interdisciplinary team, and follow Medicare, privacy, patient-rights, safety, and agency standards.

Qualifications

Active Nevada social work license appropriate to the assigned services and Medicare-qualifying education and experience are required. Home health experience is preferred. Field duties require reliable transportation, driver’s license, and auto insurance. Employment is contingent on license verification, background and exclusion screening, competency validation, and required health clearances.
