We're seeking a compassionate, motivatedPhysical Therapist to join our growing team servingMilford/Millsboro, Kent County. In this rewarding role, you'll build strong patient relationships, develop individualized treatment plans, and collaborate with an exceptional interdisciplinary team dedicated to delivering outstanding care.

What You'll Do

• Deliver skilled physical therapy services to patients in their homes
• Develop and implement individualized treatment plans
• Educate patients and caregivers on exercises, safety measures, and mobility strategies
• Collaborate with physicians, nurses, and other healthcare professionals to achieve optimal patient outcomes
• Document care accurately and in accordance with regulatory standards

Qualifications

• Minimum of1 year of experience as a Physical Therapist
• Current and appropriatePhysical Therapist licensure
• Home health or home care experience preferred
• Valid driver's license and active automobile insurance required
• Strong clinical, communication, and patient education skills
