This position is available to internal applicants only and is an opportunity for current employees to pick up additional hours outside of their regularly scheduled work hours, including evenings and weekends.

If selected and assigned to a client, you are expected to be available for your assigned schedule on an ongoing basis. If you are unable to work an assigned shift, you will be responsible for finding coverage.



All hours worked in this position will be added directly to your regular paycheck and will not be paid on a separate check.


Position Summary: 

The primary duty of the In-Home Supports (HIS) Caregiver is to provide individualized support to clients in their homes and in the community. This position works one-on-one with clients to promote independence, build life skills, work toward personal goals, and participate in activities and outings that are meaningful to them. The IHS Caregiver builds positive relationships with clients while providing dependable, person-centered support.

 

Essential Responsibilities:

  1. Assist clients with their day-to-day needs and wants.
  2. Manage assigned caseload to ensure compliance with program policies and procedures.
  3. Encourage clients to reach their personal goals and to engage in day-to-day activities, outings, and community inclusion.
  4. Be engaged throughout the day with clients in activities or community outings.
  5. Assist with coping skills to help eliminate situations that may occur between clients and peers.
  6. Assist clients in personal cares such as toileting, changing briefs, bathing, feeding, and preparing meals.
  7. Transfer clients using appropriate body mechanics and assistive equipment such as a Hoyer or manual lift, wheelchairs, Posey belt, standers, walkers, etc.
  8. Complete Pavillio, Data Sheets, and other required documentation for assigned clients.
  9. Assist in meeting the transportation needs of clients, including transporting clients to and from community activities.
  10. Assist with client medications, as assigned.
  11. Provide input in client outcomes in accordance with Minnesota Statues Person-Centered Planning and Service Delivery.
  12. Follow any client or department specific safety measures.


Qualifications: 

  1. High School Diploma
  2. Six to twelve months’ experience supporting individuals with Intellectual/Developmental Disabilities is preferred.
  3. Experience assisting clients with toileting and brief changes is preferred.
  4. Medication certification may be required based on assigned clients.
  5. Ability to use electronic devices including data bases and internet-based applications.
  6. Ability to transport clients to and from community activities in personal vehicle and meeting the minimum personal vehicle insurance requirements.
  7. Ongoing commitment to strengthening diversity, equity, and inclusion within the workplace.

  9. Ability to pass a criminal background check and drug/alcohol test following a conditional job offer as well as throughout the course of employment required by driving policies.


Physical Requirements:

The physical demands described are representative of those that must be met by an employee to successfully perform the essential responsibilities of this position. Employees must be able to frequently sit, stand, walk, and communicate throughout the workday. This position requires the ability to stoop, bend, kneel, twist, reach, push, pull, and respond quickly to client needs, including emergency situations. Employees must be able to lift and transfer a minimum of 50 pounds and assist with moving clients who may be unable to support their own weight. This includes the safe use of assistive equipment such as Hoyer lifts, standers, wheelchairs, and other adaptive devices. Employees may be required to push and pull wheelchairs and equipment supporting clients weighing 100–200 pounds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position.
