Hourly Pay Range:
$31.06 - $46.59 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position Highlights:
- Position: Medical Social Worker at Kellogg Cancer Center – MSW
- Location: Evanston Kellogg Cancer Center - Evanston, IL
- Full Time | 40 hours/week
- Hours: Monday-Friday, 8:30am - 5pm, no weekends or holidays.
- Required Travel: Swedish Hospital Cancer Center, as needed
Brief Overview:
- The Medical Social Worker will provide direct clinical social work services to Kellogg Cancer Center patients and their families as part of the Kellogg interdisciplinary team. The role of the Kellogg Social Worker is to perform psychosocial assessments on new patients to identify mood status, current coping abilities as well as to identify any practical and resource needs to lessen any barriers to them receiving their cancer care. The Kellogg Social Worker also provides ongoing emotional support and assess for practical and resource needs on an ongoing basis. The team also assists with financial needs, advance care planning and end of life conversations.
What you will do:
- Identify new chemotherapy patients and conduct a thorough initial psychosocial assessment assessing adjustment to cancer, past and present coping skills, support system, ability to engage support resources, understanding of treatment process, diagnosis and prognosis.
- As indicated, assess mental health concerns (collaborating with medical team as appropriate), substance use/abuse history, functional and cognitive impairment, health care decision making (HC-POA or guardianship), cultural differences and financial concerns/need.
- Educate patient and family on internal and community resources (including home care, transportation, mental health resources, grants, Dept of Aging/DRS, support groups, community cancer support services, etc) and facilitate patient’s connections to these resources.
- Meet with patient and family during treatment process to provide ongoing emotional support, short term counseling and assessment of practical, resource, financial needs, advocacy and care coordination.
- Provide additional supportive intervention on advance care planning, end of life goals/values and education and collaborate with medical team to facilitate appropriate referrals.
- Provide timely and effective intervention to patients referred through various referral sources, including NCCN Distress screening and consults. Collaborate with interdisciplinary team members - including radiation oncology, surgical oncology, palliative care, IP care management team.
- Provide crisis management support, assessment for safety and make appropriate reports.
- Educate team members regarding the socio-economic, emotional and cultural issues that impact the care delivery, coping skills and response to treatment.
- Document in all clinical intervention related to their cancer care withing clinical record in appropriate time frame.
What you will need:
- Education: Master’s degree in social work from an accredited school,required.
- Licensure: Illinois licensure as LCSW or LCSW-eligible, required.
- Experience: 2-3 years of social work experience in an acute hospital or health-care settingrequired. Prior experience in oncologypreferred.
- Unique or Preferred Skills: Excellent interpersonal and organizational skills.
- Demonstrated ability to communicate effectively and work collaboratively with a range of team members and patient population.
- Strong written and verbal communication skills with attention to professional documentation.
- Act as subject matter expert.
