In this role, you'll coordinate and complete Minimum Data Set (MDS) assessments, support individualized care planning, and help ensure compliance with federal and state regulations while contributing to quality resident outcomes.

Job Responsibilities

• Complete MDS 3.0 assessments and Care Area Assessments (CAAs) accurately and within required timelines.
• Maintain complete and accurate clinical documentation to support resident care planning and reimbursement.
• Ensure compliance with CMS regulations, California requirements, and facility policies.
• Collaborate with the interdisciplinary team to develop and update individualized resident care plans.
• Monitor resident progress and communicate relevant changes to the care team.
• Participate in resident care conferences and quality improvement initiatives.
• Ensure MDS submissions meet applicable Medicare and Medi-Cal documentation and reimbursement requirements.
• Assist with audits, surveys, and regulatory reviews by providing accurate documentation and reports.
• Stay current with changes to MDS requirements, regulations, and skilled nursing documentation standards.
• Educate and guide staff on documentation practices and MDS requirements.
• Communicate professionally with residents, families, and team members regarding care planning.

Qualifications

• Current, activeCalifornia Licensed Vocational Nurse (LVN) license.
