What You’ll Do
- Coordinate and complete Minimum Data Set (MDS) assessments accurately and within required timelines, in accordance with federal and state regulations.
- Develop, review, and update individualized resident care plans in collaboration with physicians, nursing staff, therapy, social services, residents, and families.
- Facilitate resident care conferences and promote effective communication among the interdisciplinary care team.
- Ensure resident records and clinical documentation accurately reflect the care provided and support appropriate reimbursement.
- Monitor residents’ conditions and coordinate follow-up to ensure their individual care needs are met.
- Assist with admissions, transfers, discharges, and timely completion of required assessments and documentation.
- Support regulatory compliance, quality assurance initiatives, and facility policies and procedures.
- Participate in leadership meetings, quality improvement activities, and other clinical initiatives as needed.
- Promote resident rights, dignity, safety, confidentiality, and a positive care environment.
- Collaborate with the nursing team to identify opportunities to improve care delivery and resident outcomes.
What We’re Looking For
- Current, active, and unencumbered RN or LPN license in the state of practice.
- Nursing degree from an accredited program.
- At least one year of experience in geriatric nursing, including medication administration and related clinical responsibilities.
- Previous MDS experience strongly preferred.
- Strong organizational skills and exceptional attention to detail.
- Excellent communication, documentation, and problem-solving abilities.
- Ability to work collaboratively with residents, families, and an interdisciplinary team.
- Flexibility, professionalism, and a genuine commitment to resident-centered care.
RN licensure is strongly preferred. LPN candidates may be considered with appropriate RN oversight.
