General Purpose
The MDS Coordinator – RN is responsible for overseeing the coordination and completion of all Minimum Data Set (MDS) documentation for patients in accordance with federal and state regulations. This role plays a critical part in care planning, reimbursement accuracy, and quality assurance within the facility.
Essential Duties
• Manage and maintain the MDS/PPS program, assessment schedule and tracking to ensure timely completion of assessments by all departments.
• Develop and coordinate nursing care plans based on individualized patient needs and monitor to ensure other departments are timely completing care plans.
• Analyze assessment data to identify complex patient needs and early clinical changes.
• Evaluate the effectiveness of care plans and update the MDS/RAI process within regulatory timelines.
• Collaborate with interdisciplinary teams to ensure comprehensive and compliant documentation.
• Maintain compliance with all applicable regulations and facility policies.
• Perform other duties as assigned to support resident care and facility operations.
Qualification Education and/or Experience
• Completion of a Registered Nursing Program
• Current RN license in good standing.
• Experience in MDS coordination and RAI process preferred.
• Strong clinical assessment and critical thinking skills.
• Familiarity with Medicare guidelines and CMG (case-mix group) classifications.
• Ability to work independently and manage multiple priorities. • Excellent communication and documentation skills.
• Proficiency in electronic health records and MDS software systems.
Physical Demands
• Ability to sit for extended periods while reviewing documentation, completing assessments, and entering data into electronic health records.
• Frequent use of hands and fingers for typing, writing, and handling medical records.
• Occasional standing, walking, bending, and reaching during patient assessments or interdisciplinary meetings.
• Must be able to visually assess residents, conduct face to face interviews and read detailed clinical documentation.
• Auditory acuity required for communicating with residents, families, and healthcare staff.
• May be required to assist with patient care activities, including repositioning or light lifting (typically under 25 pounds).
