Pleasant Hill Post Acute is seeking a compassionate, dependable, and dedicatedLicensed Vocational Nurse (LVN) to join our clinical team on aPart-Time / On-Call basis.

The ideal candidate is committed to providing safe, high-quality, resident-centered care while working collaboratively with residents, families, physicians, and the interdisciplinary healthcare team.

Responsibilities

• Provide nursing care and services to residents in accordance with established care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.
• Complete accurate and timely nursing documentation.
• Assist with assessments and contribute to individualized care planning.
• Communicate effectively with residents, families, and members of the healthcare team.
• Follow infection control, safety, and facility policies and procedures.
• Promote residents' comfort, dignity, and overall well-being.

Qualifications

• Current and validCalifornia LVN license.
• Previous skilled nursing or post-acute experience preferred.
• Strong communication and organizational skills.
• Dependable, compassionate, and team-oriented.
• Ability to work effectively in a fast-paced healthcare environment.
