Perform priority technical skills/procedures required by the area/department/assignment safely, effectively,efficiently and legally. Â
Contribute to the development of a plan of care for assigned patients.Â
Establish and/or revise priority for patient care activities based on acuity of needs, resource availability, patientpreference and other departmental needs. Â
Manage andidentify medical needs responding to dataindicating potential risk to individual's health, initiates action to correct, reduce or prevent the risk.Â
Make decisions based on scientific principles, successful experience, intuition and/or standards of practice and delegate safely and appropriately to all care team members.Â
Collaborate with care team membersidentifying actions to improve patient care and seeking supervision,consultation andassistance when unable to perform effectively or independently. Â
Use all resources to resolve and prevent problems that cannot be managed independently. Â
Maintain documentation consistent with established guidelines.Â
Maintain and advocate a safe environment for everyone. Â
Monitor and engage with your patients, families, andappropriate care team members concerningsignificant events, patient condition changes, or patient questions and concerns.Â
Ensure continuity of care, efficient throughput, consistent communications, and an exceptional experience for every patient.Â
Initiate and/ormaintain interventions to assure safe and effective care including medication management.Â
Educate patients and family usingappropriate resources and continuum of care services.Â

Minimum:Â
Licensed Practical Nurse (LPN) --Louisiana State Board of Practical Nurse Examiners  Â
Current American Heart Association BLS certification.Â
