We are seeking a dedicated and person-centered Program Specialist to coordinate services and supports for individuals participating in our Adult Day programs. This role serves as a key member of the interdisciplinary team, ensuring Individual Support Plans (ISPs) are implemented effectively, regulatory requirements are met, and individuals receive high-quality services that support independence, choice, and community integration.

What You'll Have:
- One of the following qualifications:
- Master's degree and one year of experience working directly with persons with disabilities; or
- Bachelor's degree and two years of experience working directly with persons with disabilities; or
- Associate degree, 60 college credits, or completion of a two-year program and four years of experience working directly with persons with disabilities.
- For unlicensed programs, five years of experience in the human services field may be substituted for educational requirements.
- Experience with electronic health record systems, such as CareLogic, preferred.
- Strong record management and documentation skills.
- Valid driver's license required.
- Ability to complete CESP or ACRE-approved Basic Employment Services training within nine months of hire, if required.
- Act 33 and Act 34 clearances required; FBI clearance required if applicable.
A Typical Day May Include:
- Coordinating services and supports for assigned individuals and ensuring ISP goals are implemented effectively.
- Participating in ISP meetings, assessments, quarterly reviews, admissions, and discharge planning.
- Reviewing documentation for compliance, accuracy, quality, and timeliness.
- Monitoring progress toward individual goals and ensuring services align with personal interests and preferences.
- Training and supporting staff on ISP implementation, behavior plans, and individual-specific supports.
- Tracking regulatory due dates and ensuring required documentation is completed on time.
- Maintaining and updating electronic records within CareLogic.
- Monitoring health, safety, rights, and well-being of individuals receiving services.
- Collaborating with Operations Supervisors, families, support coordinators, and other service providers.
- Assisting with incident reporting, investigations, and follow-up activities as needed.
- Collecting and analyzing program data and preparing reports to support program improvement.
- Supporting community integration, employment initiatives, and meaningful day programming opportunities.
- Participating in committees, quality improvement activities, and annual training requirements.
