Service Coordinator Position Summary:
The Service Coordinator is responsible for assuring that the Service Plan is appropriately implemented. The Service Coordinator assists the prospective participant to become a TBI waiver participant and coordinates and monitors the provision of all services in the Service Plan. Services may include Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal of the Service Coordinator is to increase the participant’s independence, productivity and integration into the community while maintaining the health and welfare of the individual.

Service Coordinator Responsibilities:
- Provides advocacy, referrals and service coordination/case management to individuals who are enrolled in the TBI waiver.Â
- Assures continuity of participant services and high standards of participant care; facilitates individualized service planning and implementation of goals in cooperation with the treatment team.
- Maintains and updates required records and documents services provided. Reports relevant observation of individual behaviors, progress and reactions.Â
- Works closely with significant people and organizations in each individual’s life to ensure appropriate team support and monitor satisfaction of services as stated in the Individualized Service Plan.

Service Coordinator Qualifications:
- Service Coordinators must be a Master of Social Work; Master or Doctorate in Psychology; Registered Physical Therapist; Registered Professional Nurse; Certified Special Education Teacher; Certified Rehabilitation Counselor; Licensed Speech-Language Pathologist; Registered Occupational Therapist. Â All providers must have a minimum of one year of experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; ORÂ
- Bachelor’s degree in Human Services field and two years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; ORÂ
- Associate’s degree and three years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; ORÂ
- High School Diploma or equivalent and four years’ experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; ORÂ
- Be an individual who has successfully served as a Regional Resources Development Specialist for one year in the HCBS/TBI Waiver ProgramÂ

Pay Rate: $25.50 per hour
