Position Summary

Ridgeway Post Acute is seeking a compassionate and motivatedPhysical Therapist Assistant (PTA) to join our rehabilitation team. Under the direction and supervision of a licensed Physical Therapist, the PTA will provide high-quality therapeutic interventions to help residents regain strength, mobility, independence, and quality of life.

Essential Duties & Responsibilities

  • Provide physical therapy treatments as directed by the supervising Physical Therapist and/or Director of Rehabilitation.
  • Document resident treatments, progress notes, and outcomes accurately and timely in accordance with facility policies, Medicare guidelines, and regulatory requirements.
  • Maintain a safe treatment environment by adhering to infection control, safety, and equipment maintenance standards.
  • Participate in care plan meetings, resident care conferences, and rehabilitation team meetings as needed.
  • Assist with the training and education of Restorative Nursing Aides and other team members.
  • Communicate regularly with the supervising Physical Therapist regarding resident progress, treatment goals, physician updates, discharge planning, and interdisciplinary coordination.
  • Educate residents, families, and caregivers on therapy goals, home exercise programs, safety techniques, fall prevention, and maintenance programs.
  • Recommend and facilitate the procurement of appropriate durable medical equipment (DME), including walkers, canes, splints, grab bars, elevated toilet seats, and other adaptive devices.
  • Collaborate effectively with physicians, nursing staff, therapists, residents, and families to support optimal outcomes.
  • Maintain resident confidentiality and comply with HIPAA and facility policies at all times.

Qualifications

Required

  • Current and unrestrictedPhysical Therapist Assistant (PTA) license
  • Graduate of an accredited Physical Therapist Assistant program
  • Strong communication and interpersonal skills
  • Ability to work effectively within a multidisciplinary healthcare team
  • Current CPR certification (preferred if required by state regulations)

Preferred

  • Experience in aSkilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care setting
  • Knowledge of Medicare regulations, therapy documentation requirements, and rehabilitation best practices
  • Experience working with geriatric and medically complex patient populations
