As the RN Care Manager & Patient Advocate, you will serve as a trusted advisor and advocate for older adults and their families. You will use your nursing expertise to help clients navigate healthcare decisions, coordinate care, and maintain their independence, health, and safety. You will:

• Build trusted relationships with clients and their families
• Conduct in-home assessments and ongoing care evaluations
• Coordinate care and communicate with healthcare providers
• Attend medical appointments as a client's advocate
• Monitor medications and treatment plans
• Help clients navigate healthcare and community resources
• Collaborate with home health and caregiving teams
• Identify opportunities to improve client safety, well-being, and quality of life
• Serve as a primary point of contact and trusted resource for clients and families

To be successful in this role, you will have:

• Associate's or Bachelor's degree in Nursing
• Active Ohio Registered Nurse license
• 10+ years of nursing experience inICU, Geriatrics, Hospice, Home Health, Case Management, or Care Management preferred.
• Current BLS certification preferred
• Strong clinical judgment and assessment skills
• Excellent communication and relationship-building abilities
• Experience coordinating care and working with interdisciplinary teams
• Proficiency with Microsoft Office and electronic medical record systems
• Ability to handle confidential and sensitive information with professionalism and discretion
• Passion for helping older adults maintain independence, health, and safety
