Social Worker – Disabilities

Liquid Personnel is recruiting anAdult’sSocial Worker to join its client’s Disabilities team, based in Powys. 

What will your responsibilities be? 

• Managing a caseload of adults with physical disabilities, sensory impairments, learning disabilities, long-term health conditions, and complex care and support needs.
• Undertaking strengths-based assessments in accordance with the Social Services and Well-being (Wales) Act 2014.
• Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and positive outcomes.
• Working collaboratively with individuals, carers, families, advocacy services, health professionals, housing providers, and partner agencies.
• Undertaking safeguarding enquiries and responding appropriately to concerns relating to abuse, neglect, exploitation, and self-neglect.
• Completing Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.
• Assessing and managing complex risks whilst promoting positive risk-taking and individual choice.
• Supporting adults through significant life changes, transitions, accommodation issues, and changes in support arrangements.
• Coordinating services and support packages to ensure individuals receive timely and effective interventions.
• Promoting access to community resources, direct payments, preventative services, and support networks that enhance independence.
• Participating in multidisciplinary meetings and contributing to integrated care and support planning.
• Preparing high-quality assessments, reviews, reports, and professional documentation.
• Maintaining accurate, timely, and legally compliant case records in accordance with organisational requirements. 

Qualifications and Experience: 

To be successful in this role you must have;  

• Professional Qualification: Degree in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Care Wales.
• Experience: Significant post-qualified experience within Adult Social Care services.
• Disabilities Experience: Experience supporting adults with physical disabilities, learning disabilities, sensory impairments, and complex care needs.
• Legislation Knowledge: Strong understanding of the Social Services and Well-being (Wales) Act 2014, Mental Capacity Act 2005, Human Rights legislation, and Adult Safeguarding procedures.
• Assessment Skills: Proven ability to undertake strengths-based assessments, reviews, care and support planning, and risk management interventions.
• Safeguarding Experience: Experience managing safeguarding concerns and undertaking safeguarding enquiries involving vulnerable adults.
• Multi-Agency Working: Strong experience working collaboratively with health professionals, housing providers, carers, advocacy organisations, and partner agencies.
• Communication Skills: Excellent communication, report-writing, assessment, analytical, and decision-making skills.
• Professional Practice: Commitment to strengths-based, person-centred practice with a focus on independence, wellbeing, and inclusion.
• Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
