Liquid Personnel is recruiting anAdult’sSocial Worker to join its client’s Community Older People's team, based in Solihull.This is an excellent opportunity for a dedicated practitioner to support older adults within the community, promoting independence, wellbeing, and positive outcomes through strengths-based and person-centred social work practice.

The successful candidate will manage a varied caseload of older adults with complex care and support needs, undertaking statutory assessments, reviews, safeguarding interventions, and support planning. Working collaboratively with service users, carers, families, health professionals, and partner agencies, you will play a key role in ensuring individuals receive appropriate support to remain safe, independent, and connected to their communities.

This role requires a confident Social Worker with strong assessment skills, sound knowledge of Adult Social Care legislation, and the ability to manage complex casework whilst delivering high-quality, outcome-focused interventions.

What will your responsibilities be? 

  • Managing a caseload of older adults with a range of care and support needs within a community setting.
  • Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and promote positive outcomes.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
  • Arranging and coordinating care packages, community support services, and support arrangements to meet assessed needs.
  • Undertaking safeguarding enquiries and interventions in line with Adult Safeguarding legislation, policies, and procedures.
  • Completing Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Assessing and managing risk effectively, balancing individual choice with safety and safeguarding responsibilities.
  • Supporting adults and carers through periods of change, providing advice, advocacy, guidance, and access to appropriate services.
  • Working collaboratively with health professionals, community services, care providers, carers, families, and partner agencies to achieve positive outcomes.
  • Preparing professional reports, assessments, and documentation for legal proceedings, including Court of Protection matters where required.
  • Participating in multidisciplinary meetings, safeguarding discussions, case reviews, and professional forums.
  • Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.

Qualifications and Experience: 

To be successful in this role you must have;  

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care services.
  • Older People's Services Experience: Experience supporting older adults within a community-based Adult Social Care setting.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake Care Act Assessments, reviews, risk assessments, and support planning.
  • Safeguarding Experience: Experience undertaking safeguarding enquiries and managing complex safeguarding concerns.
  • Court Work: Experience preparing professional reports and supporting Court of Protection processes is desirable.
  • Multi-Agency Working: Strong experience working collaboratively with health professionals, community services, care providers, families, and carers.
  • Communication Skills: Excellent communication, report-writing, negotiation, and decision-making skills.
  • Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
  • Organisational Skills: Ability to manage a complex caseload effectively and work independently within a busy community team.
