Social Worker - Review Team

Liquid Personnel is recruiting anAdult’sSocial Worker to join its client’s Review team, based in East Riding of Yorkshire.

What will your responsibilities be? 

• Complete strengths-based reviews and reassessments of adults with care and support needs, ensuring outcomes remain person-centred and compliant with the Care Act 2014.
• Review existing care packages, direct payments and support plans, making recommendations to promote independence, manage risk and achieve value for money.
• Work collaboratively with service users, families, providers and partner agencies to assess changing needs, coordinate services and ensure timely interventions.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
