Social Worker - Learning Disability

Liquid Personnel is recruiting anAdult’sSocial Worker to join its client’s Learning Disability team, based in Leeds.

What will your responsibilities be? 

• Completed specialist assessments for adults with learning disabilities, identifying care and support needs, promoting independence, and developing person-centred care plans in line with the Care Act 2014.

• Managed a diverse caseload, undertaking reviews, safeguarding enquiries, Mental Capacity Act assessments, and risk assessments to ensure the safety and wellbeing of service users.

• Worked in partnership with healthcare professionals, families, providers, and community agencies to coordinate effective support packages, facilitate transitions, and improve outcomes for individuals with learning disabilities.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
