Liquid Personnel is recruiting anĀ AdultSocial Worker to join its clientās Community / Supported Living team, based inĀ East Riding of Yorkshire Council.
What will your responsibilities be?Ā
Conducting Care Act reviewsĀ ā completing ināperson Care Act assessments, updating support plans and ensuring support remains appropriate, strengthsābased and costāeffective.
Monitoring safety and wellbeingĀ ā supporting ISL tenants to stay safe, well and independent, identifying risks early and liaising closely with Futures+ and Commissioning teams.
Applying safeguarding and MCA/DoLS practice ā undertaking mental capacity assessments, contributing to bestāinterest decisions and responding to safeguarding concerns within supported living settings.

Qualifications and Experience:Ā
To be successful in this role you must have; Ā
- Social Work England registrationĀ
- Eligible to work in the UKĀ
- Hold a full UK driving licenceĀ
- Post-qualified experienceĀ
- Degree level or equivalent in Social Work Ā Ā
