Job Summary:
The Care-Coordination Team Leadis responsible for ensuring the delivery of high-quality healthcare by owning end-to-end referral operations for Ennoble Care's partnership with a national home health provider. At launch, this is a hands-on operational role with direct responsibility for intake: the Team Lead personally performs all intake activities for partnership referrals — working the referral intake queue daily, verifying that referral data is complete, performing insurance verification, assigning providers, and advancing patients through the intake pipeline — in addition to coordinating outbound referrals and serving as a primary point of contact for partner communication. As the partnership scales, the Team Lead will build and lead a team of Care Coordinators supporting partnership markets, communicate company goals, safety practices, and deadlines to the team, motivate team members and assess their performance, and keep management updated on team performance.Â
Responsibilities:
Referral intake & processing:Â
- Own the home health partnership referral processing queue end to end, serving as the dedicated owner for all inbound partnership referralsÂ
- Work the shared care coordination platform's intake queue daily: confirm all referral data is present and complete, perform insurance verification, create the patient chart in the EMR, assign a provider where needed, and advance each patient through pipeline stages with documented reasons for any patient who does not proceedÂ
- Manage the dedicated partnership inbox, resolving missing-information questions, insurance questions, and duplicate referralsÂ
- Acknowledge every referral received and monitor form submissions dailyÂ
- Maintain the referral log, ensuring no referral is lostÂ
Outbound referral coordination:Â
- Serve as centralized coordinator for outbound home health referrals: create referrals in the chart, send them to the partner market contact, and reroute rejected referrals with documented reasonsÂ
- Handle documentation exchange, including clinical-note requests, 485/order sign-offs, and routing orders to collaborating physicians where requiredÂ
- Confirm start of care and document it on the patient chartÂ
- Review referral-routing exceptions on a regular basisÂ
Partner communication & reporting:Â
- Staff the partnership's clinical escalation channels: urgent issues by phone within a defined response window, routine issues with same-day acknowledgment and loop closureÂ
- Manage partnership patient communication, urgent clinical issues, and order/portal workÂ
- Maintain account-level reporting and the referral funnel, reviewing dailyÂ
- Support end-to-end testing ahead of launch and participate in daily launch-period check-insÂ
Team leadership (potential future state)
Qualifications:
- At least 2 years of experience in a healthcare operations, care coordination, or referral management setting; management experience strongly preferredÂ
- CMA or LPN license preferredÂ
- Valid Driver's license and access to a vehicle preferredÂ
- Strong proficiency with healthcare operations systems (EMR, CRM, and intake platforms); comfortable learning new tools quicklyÂ
- Knowledge of principles and processes for providing customer and patient services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfactionÂ
- Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resourcesÂ
- Proficiency in the structure and content of the English language, including the meaning and spelling of words, rules of composition, and grammarÂ
- Understanding of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effectsÂ
- Familiarity with principles and procedures for personnel managementÂ
Starting Salary: $50,000 per year + bonus structure
