JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
The Male Community Reentry Program (MCRP), a program of HealthRIGHT 360, is a residential facility with a capacity of 200 beds, designed for adult males who have less than two years remaining on their prison sentences. Instead of being incarcerated in a state prison, eligible individuals can complete the remainder of their sentences in the community through MCRP. The program offers a variety of rehabilitative services focused on substance use disorder (SUD), mental health care, primary care, employment, education, housing, family reunification, and social support. Treatment for substance use disorders and mental health issues provided by MCRP typically lasts from 6 to 12 months, ensuring comprehensive support for participants.
The Intake Coordinator conducts the initial evaluation of potential clients to assess their requirements for SUD services, completes admission paperwork, and creates the initial Treatment Plan. This in-person role ensures a smooth transition for clients to their treatment team, stabilizing new clients and addressing immediate needs. The Intake Coordinator I collaborates closely with counselors, case managers, therapists, and other team members to organize and oversee client concerns, make necessary referrals, and offer educational support.
