Job Description:

Baptist Health Rehab Servicesprovides a full range of physical, occupational, and speech therapy services for all ages. Our therapists and assistants are trained in treating neurological, orthopedic, vestibular, lymphedema, pediatrics, industrial medicine, swallowing, and speech and language disorders. 

The staff physical therapist shall evaluate and treat patients, develop a treatmentplan, and set goals forpatients. Patient and family educationis included in the treatment plan and goals. The PT will completedocumentation according to applicable national and state regulations as well as hospital and departmentalpolicies. The therapist willassist with the education of affiliating students, department staff, hospital staff,and the community at large. The therapist will appropriately delegate to and supervise physical therapistassistants and technicians. The therapist willparticipate indepartmental operational plan.The therapistmust intervene appropriately and meet the needs of patients with impaired mental abilities, vision, hearing, or mobility.

Full-Time, Days 

Job Description: 

• Completesappropriate chart / medical record review. 

• Followsplan of treatment appropriately, notifiesreferring provider of change in status and need formodifications to treatmentplan or goals. 

• Plans treatment appropriately, including input frompatient. 

• Receives and implements therapy orders. 

• Completes evaluation form, treatment notes, progress reports, and D/C summariesin a timelymanner, and appropriatelysubmits toreferring provider. 

• Supervises/delegates/directs activities of students and Rehab Services Technicians. 

Requirements: 

• Bachelors Degree in Physical Therapy 

• Current license by the Ky Board of Physical Therapy 

• BLS within30 days from date of hire
