Summary/ObjectiveÂ
To assist families experiencing homelessness in acquiring full Emergency Assistance (EA) eligibility. The PSS Case Manager will support families in working with their assigned Homeless Coordinator to identify outstanding eligibility criteria, including but not limited to collecting documents and verifications. The Lead Case Manager is responsible for liasioning with EOHLC staff regarding Priority Site operations and processes, specific case inquires, and ongoing communication. As with all shelter staff, the Lead Case Manager will assist in the overall maintenance of a structured and enhanced shelter environment.

Essential Functions
- Assist with training Case Managers and Support Staff in all areas of the Priority Support Site.
- Assist with after hours on call responsibilities.
- Assist VPHS in monitoring all program requirements.
- Daily meetings with Case Managers to review and prioritize family roster and unit inventory.
- Meet with each new family within one business day of arrival to shelter. Initial meeting will include but is not limited to completing the PSS intake, review and obtain signatures for Uniform Shelter Rules, Program Rules, License and Participation agreement, and all other applicable program documents.
- Ensure families have adequate food and general hygiene products.
- Provide orientation to facility and thorough overview of emergency procedures.
- Assist families with their EA family shelter program application. Work with the assigned Homeless Coordinator to identify outstanding eligibility criteria; this includes corresponding via email and phone. Support families in gathering required documents and uploading them in the HHH system.
- Comply with all EOHLC requirements in the ETO system. Utilize the ETO data system to record entry/exit to shelter.
- Document case management efforts and meetings utilizing the ETO case notes system.
- Monitor cleanliness of units by doing unit inspections. Address any health and safety concerns with shelter guest.
- Report maintenance issues to admin support staff in a timely manner.
- Clean unit after family exits shelter ensuring unit is health and safety ready for a new family arrival.
- Prepare each unit for new move-in taking steps necessary to have unit clean, orderly, and ready for new family.
- Attend EOHLC and PSS meetings as required.
- Attend staff meetings as required
- Provide transportation as needed.
- Maintain privacy of guest information in accordance with agency policies, procedures, practices, and as required by law.
Additional Duties
- Call case conferences as needed and include all necessary parties.
- Assist other team members as needed or as directed by the supervisor.
- Other duties as assigned by supervisor or to help meet program criteria.
- Participate in training required by Housing Assistance Corp or EOHLC
- Comply with all reporting requirements of the shelter funding source, including maintaining accurate case files.

Required Qualifications, Education, and Experience
- HS Diploma or equivalency
- 2 years experience working with homeless or low income families
- Experience working with clients in crisis
- Strong ability to effectively resolve and cope with immediate crisis situations
- Detail oriented with strong organizational and time management skills
- Demonstrated ability to work independently and as part of a team
- Able to set and observe appropriate boundaries with clients
- Possess well developed decision making skills
- Ability to multi-task and remain calm in demanding and unpredictable situations
- Computer skills including MS Word and Microsoft Outlook
- Valid MA Driver’s license

- Bachelor’s degree in Human Service or Social Science field
- 5+ years experience working with homeless or low income families
- Knowledge of community resources
- Ability to work effectively with diverse groups of people
- Bilingual
