Home Health Registered Nurse

Responsibilities

• Conduct patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Monitor and document patient conditions and progress
• Provide wound care, catheter care, and other skilled nursing services
• Educate patients and family members on health conditions and care plans
• Coordinate care with physicians, therapists, and other healthcare professionals
• Maintain accurate and timely documentation in compliance with agency and state regulations

Qualifications

• Active RN or LPN license in the state of Indiana
• Minimum 1 year of nursing experience (home health experience preferred)
• Current CPR certification
• Valid driver’s license and reliable transportation
• Strong clinical assessment and documentation skills
• Excellent communication and interpersonal skills
• Ability to work independently
